You've Built Your Practice. Now What?
There’s a particular moment in a clinician’s career that doesn’t get talked about enough.
You’re licensed. You’re experienced. You’re seeing clients. Maybe your caseload is full or close to it. From the outside, it looks like you’ve made it.
But internally, you may find yourself asking: What’s next?
For Dr. Viviana Coles, that question represents an important transition in an MFT’s professional journey. Once you’ve established yourself as a capable clinician, growth isn’t necessarily about doing more. It’s about becoming more intentional about what you want to be known for, how you want to work, and what you want your professional future to look like.
In Beyond Solo Practice: Advanced Growth Pathways for MFTs, Dr. Coles draws on her experience as a successful clinician, practice owner, supervisor, and speaker to explore how experienced clinicians can move beyond simply maintaining a successful practice and begin intentionally designing what comes next.
From busy to distinct
One of Dr. Coles’ most powerful lessons comes from her own early career. When she first began seeing clients in graduate school, she believed being a great therapist meant being able to help everyone. She said yes to every presenting issue, every couple, every individual, and every opportunity.
The result? She was busy—but she wasn’t building anything that felt distinct.
Over time, she realized that trying to do everything made it difficult for others to understand what made her work different or why they should specifically refer to her. That experience led to an important shift: specialization isn’t about limiting your clinical ability. It’s about focusing your professional identity.
You don’t have to do everything—but you should understand it
As practices grow, outsourcing can become an important part of building sustainable systems. But Dr. Coles cautions against outsourcing areas of your business before you understand how they work yourself. You may eventually hire someone to manage your website, marketing, or other business functions. But you should understand those pieces well enough to know what is working, what isn’t, and what decisions need to be made. As she puts it, you don’t need to do everything forever, but you do need to understand everything at least once.
Growth requires action, not perfect readiness
Expanding your professional identity can also bring uncertainty. You may want to pursue speaking opportunities, increase your visibility, write, teach, or explore new ways of creating impact—but hesitate because you don’t feel quite ready. Dr. Coles challenges that mindset.
Confidence doesn’t come before action. It comes from experience.
Taking aligned opportunities, showing up, and learning in real time can help you grow into the next version of your professional identity. You don’t have to wait for perfect timing—because, as Dr. Coles points out, it doesn’t exist. And that may be the most important message for clinicians standing at this particular crossroads: you don’t have to have every detail of your next chapter figured out before you begin building it.
